Digital copies of your records from Mount Baker Rheumatology can be requested by filling out the secure release form on this site — click the “Request My Records” button to get started. In accordance with applicable state law, a reasonable fee may be charged for the copying and delivery of records. You will receive an email confirmation once your request is received.

In accordance with applicable state law, you may be charged a reasonable fee for the copying and transmittal of records. The exact amount depends on the size of your file and your state's regulations.
Most states allow by law up to 30 days to process records requests. Due to the high volume of requests we receive, it can take up to 30 days. We do our best to process all requests sooner, as we know how important they are.
Yes, with proper authorization. Parents or guardians for minors, attorneys with a HIPAA-compliant release form, and others with notarized permission may submit requests on your behalf. The form will guide you through what is required.
